Understanding the layered anatomy of the face is critical for safe filler placement. Unlike botulinum toxin which targets muscles, fillers are placed at various depths depending on the desired effect. I'll teach you to visualize facial layers, fat compartments, and danger zones.
Depth: 1-4 mm
Filler Use: Fine lines, superficial wrinkles (thin, low G' fillers)
Depth: Variable (4-15 mm)

Safety Alert
Vascular occlusion is the most serious complication of filler injection. Memorize these danger zones and always aspirate before injecting.
Vessel: Supratrochlear artery
Risk: Retrograde flow to ophthalmic artery → vision loss
Vessels: Dorsal nasal, lateral nasal, angular arteries
Risk: Nasal necrosis, vision loss via anastomoses
Vessels: Facial artery, angular artery
Risk: Inject superficially, aspirate, small aliquots

I always recommend injecting in the safest plane for each area. When in doubt, go more superficial — you can always add more filler, but you can't undo vascular occlusion.

I prefer cannulas for high-risk areas like tear troughs and nasolabial folds. The blunt tip reduces vascular injury risk significantly.
Safety Alert
Always aspirate before injecting, especially in danger zones. If you see blood, withdraw and apply pressure. Use small aliquots (≤0.1 mL) and inject slowly.

I always approach the face holistically. Often, treating one area (like cheeks) reduces the need for filler in adjacent areas (like nasolabial folds). This creates more natural results with less product.
This is the most critical section of your training. Vascular occlusion is a medical emergency that requires immediate recognition and treatment. I require all my students to have hyaluronidase available and know exactly when and how to use it.
Signs: Immediate blanching, severe pain disproportionate to injection, dusky/mottled skin, slow capillary refill
Safety Alert
If you suspect retinal artery involvement (vision changes, eye pain), this is an EMERGENCY. Inject hyaluronidase immediately and arrange urgent ophthalmology consultation. Every minute counts.
Products: Hylenex, Vitrase (150 units/mL)
Ice, arnica, avoid blood thinners. Resolves 1-2 weeks.
Massage if early. Hyaluronidase if persistent. May need excision.
Blue discoloration from superficial placement. Hyaluronidase to dissolve.
Rare. Antibiotics, warm compresses. May need I&D if abscess.
Delayed nodules. Intralesional steroids, 5-FU, or excision.
