Facial Muscle Anatomy for Botulinum Toxin Injection

Understanding facial anatomy is the foundation of safe and effective neurotoxin treatment. I recommend studying the origin, insertion, and action of each muscle thoroughly — this knowledge will guide your injection placement and help prevent complications.

Upper Face
Frontalis

Action: Elevates eyebrows, creates horizontal forehead lines

Origin: Galea aponeurotica

Insertion: Skin of eyebrows and root of nose

Injection Depth: Intradermal to superficial subcutaneous

Corrugator Supercilii

Action: Draws eyebrows medially and downward (frown)

Origin: Medial supraorbital ridge

Insertion: Skin above middle of eyebrow

Injection Depth: Intramuscular, deeper injection

Procerus

Action: Pulls medial brow down, creates horizontal nasal root lines

Origin: Nasal bone and lateral nasal cartilage

Insertion: Skin between eyebrows

Injection Depth: Superficial, single midline injection

Periorbital Region
Orbicularis Oculi

Action: Closes eyelids, creates crow\'s feet

Parts: Orbital, palpebral, lacrimal portions

Origin: Medial orbital margin, medial palpebral ligament

Injection Depth: Intradermal, lateral to orbital rim

Dr. MTAI

Safety Alert

Never inject within 1 cm of the orbital rim. The levator palpebrae superioris lies deep to this area — affecting it causes eyelid ptosis, one of the most distressing complications for patients.

Mid & Lower Face
Nasalis

Action: Compresses/dilates nostrils (bunny lines)

Injection: Superficial, along nasal sidewall

Orbicularis Oris

Action: Closes lips, creates perioral lines

Injection: Very low doses, vermilion border

Masseter

Action: Elevates mandible (chewing)

Use: Jaw slimming, bruxism treatment

Injection: Deep intramuscular, lower masseter

Platysma

Action: Depresses mandible, creates neck bands

Injection: Along visible bands when contracted

Dr. MTAI

Dr. MTAI's Tip

I recommend practicing muscle identification on yourself in the mirror. Contract each muscle group and palpate to understand the relationship between surface landmarks and underlying anatomy.

Botulinum Toxin Dosing Reference Guide

Proper dosing is both a science and an art. The doses I've compiled here are based on FDA guidelines and clinical best practices. Remember: it's always better to start conservatively — you can add more at follow-up, but you can't take away what's already been injected.

Note: Doses shown are for onabotulinumtoxinA (Botox®). Adjust for other products using conversion ratios.

Treatment Area Typical Dose Range Injection Points Units per Point Key Considerations

  • Duration: 6-9 months
Dr. MTAI

Dr. MTAI's Tip

These are general guidelines. I always individualize dosing based on muscle mass, treatment history, gender, and desired outcome. For new patients, I recommend starting at the lower end of the range — you can always add more at the two-week follow-up.

Treatment Timeline & Patient Expectations

Setting proper expectations is crucial for patient satisfaction. I always walk patients through this timeline during consultation so they understand when to expect results — and when NOT to panic if they don't see immediate changes.

Day 0
Treatment Day
  • Injection procedure takes 10-15 minutes
  • Mild redness, swelling at injection sites (resolves in hours)
  • Small risk of bruising
  • Instructions: No rubbing, remain upright 4 hours, avoid strenuous exercise 24 hours
Day 2-3
Initial Onset
  • First effects may become noticeable
  • Muscle movement begins to decrease
  • Dysport and Jeuveau may show earlier onset
  • Effect may appear asymmetric initially (normal)
Day 7
Significant Effect
  • Most patients see noticeable improvement
  • Wrinkle reduction visible at rest
  • Do NOT assess for touch-up at this point — too early
Day 14
Peak Effect ✓
  • Optimal time for follow-up assessment
  • Full effect achieved
  • Appropriate time to assess for touch-ups if needed
  • Take post-treatment photos
Wk 8-10
Effect Plateau
  • Muscle movement begins to return
  • Effect still significant but waning
  • Patient may notice increased movement
Mo 3-4
Effect Diminishing
  • Near-full muscle function returns
  • Optimal re-treatment window: before full return of movement
  • Regular treatments may extend duration over time
Dr. MTAI

Dr. MTAI's Tip

I tell my patients who maintain regular treatments every 3-4 months that they often experience longer-lasting results over time. The muscles gradually weaken from reduced use — some patients eventually need less product or fewer treatments.

Dr. MTAI

Safety, Complications & Management

This is the most critical section of your training. Patient safety must always be your top priority. I require all my students to master contraindications and complication management before performing any hands-on injections. Take your time here — this knowledge protects your patients and your practice.

Absolute Contraindications
Dr. MTAI

Safety Alert

Never proceed with treatment if any of these conditions are present. There are no exceptions.

  • Known hypersensitivity to botulinum toxin or formulation components
  • Infection at injection site
  • Pregnancy and breastfeeding
  • Neuromuscular disorders (myasthenia gravis, Lambert-Eaton syndrome, ALS)
  • Aminoglycoside antibiotics (potentiates effect)
Relative Contraindications / Precautions
  • Bleeding disorders or anticoagulant therapy
  • Unrealistic expectations
  • Body dysmorphic disorder
  • History of dysphagia or aspiration
  • Marked facial asymmetry
  • Previous facial surgery affecting target area
Common Complications & Management
Complication Incidence Cause Prevention Management
Bruising 10-25% Vessel injury
  • Avoid blood thinners 7-10 days
  • Apply pressure post-injection
  • Use smaller gauge needle
Ice, arnica; resolves 7-14 days
Headache 1-10% Unknown; injection trauma Gentle injection technique OTC analgesics; resolves 24-48 hours
Eyelid Ptosis 1-5% Toxin migration to levator palpebrae superioris
  • Stay ≥1 cm above orbital rim
  • Avoid excessive volume
  • Patient remains upright
Apraclonidine 0.5% eye drops 1-2 drops TID
(stimulates Müller's muscle; raises lid 1-2mm)
Resolves in 2-4 weeks
Brow Ptosis 2-5% Over-treatment of frontalis without glabellar treatment
  • Always treat glabella with forehead
  • Conservative forehead dosing
  • Stay ≥2 cm above brow
Wait for effect to wear off (6-8 weeks); small amount of toxin to lateral brow depressors may help
Asymmetry 2-5% Uneven dosing, existing asymmetry, variable muscle activity Careful assessment, pre-treatment photos Touch-up at 2 weeks with additional units to weaker side
"Spock Brow" 1-3% Under-treatment of lateral frontalis Treat lateral forehead adequately Add 2-4 units to lateral brow area
Diplopia Rare Migration to extraocular muscles Proper injection depth, distance from orbit Supportive care; resolves with toxin wearing off

FDA Black Box Warning

The effects of botulinum toxin may spread from the area of injection to produce symptoms consistent with botulinum toxin effects. These symptoms have been reported hours to weeks after injection. Swallowing and breathing difficulties can be life threatening and there have been reports of death. The risk of symptoms is probably greatest in children treated for spasticity but symptoms can also occur in adults, particularly in those with underlying conditions that would predispose them to these symptoms.

Dr. MTAI

Dr. MTAI's Tip

I keep apraclonidine 0.5% eye drops in my emergency kit at all times. If eyelid ptosis occurs, early treatment can significantly improve patient comfort while waiting for the toxin effect to wear off.
