What this track covers
This track covers the four conditions that dominate primary care panels: type 2 diabetes, hypertension, COPD, and heart failure. It focuses on matching therapy to disease phenotype and patient factors, recognizing when a regimen needs escalation, and coordinating overlapping comorbidities in patients who often have more than one of these conditions at once.
